HI-BRIDGE Health Information Exchange (HIE) is a mission-based Regional Health Information Exchange (formerly Georgia HealthConnect) created to provide interoperability through integrated technology and clinical support services to meet the needs of smaller practices, hospitals and health systems for the electronic exchange of patient clinical information.
Admission, Discharge and Transfer notifications are sent to the patient's primary care physician and care team when a patient experiences a change in status during a hospital stay. These alerts play an integral role in ensuring proper care coordination and follow-up care, while also helping to prevent unnecessary readmissions. HI-BRIDGE HIE receives ADT notifications from GaHIN-contributing hospitals and health systems in Georgia and securely routes them to our members in real-time.
HI-BRIDGE HIE allows health professionals to query and retrieve a patient’s medical information at the time of patient care for enhanced access to comprehensive, up-to-date clinical patient data from HIE connected healthcare organizations. This exchange supports continuity of care, enhances clinical decision-making, and improves care coordination across the patient’s healthcare journey.
